To make an appointment, please have one of your doctors make a referral to our clinic. It is preferable that the doctor currently treating your pain problem makes this referral to us.
Please complete new patient form and return it along with all pertinent medical information including MRI, X-Rays, blood tests, and list of all medications to our office.
