FOR OUR REFERRING PHYSICIANS

 

We would like to make working with Pacific Pain Treatment Center (PPTC) as easy as possible for you. There are three ways in which we can assist you:

• Comprehensive evaluation and treatment by PPTC
• A pain consultation only and your patient is referred back to you for follow-up
• A specific procedure that you request and that PPTC provides

To ensure that we schedule your patient quickly and appropriately, please:

• Complete the physician referral form and fax it to our office
• Obtain any necessary initial authorizations for HMO, WC
• Fax to us all pertinent information, including MRI and X-ray reports, lab data, prior treatments, and medications
• Instruct your patient to bring radiographic films with them to the first appointment

If your patient has an emergent pain problem and you would like the immediate attention of a pain specialist:

• Please call our office and have the receptionist page us
• We make every attempt to see urgent consults within 24 hours
